INSURANCE & BENEFIT VERIFICATION
Life
Stone Counseling Centers accepts all in-network insurance plans,
subject to individual provider credentialing and network participation.
Not all providers at Life Stone may be contracted with every insurance
plan,
and network participation may vary by provider, location, or service
type. Clients are responsible for confirming that their assigned
provider is in network with their specific insurance plan.
Insurance
benefits—including but not limited to pre-authorization requirements,
deductibles, copayments, coinsurance, session limits, coverage
exclusions, and medical necessity determinations—are governed solely by
the terms of each client’s individual insurance policy.
As
a courtesy, Life Stone offers complimentary insurance benefit
verification prior to the initiation of services. Any information
provided regarding insurance benefits is based solely on information
obtained from the insurance carrier at the time of inquiry and is
provided as an estimate only.
Insurance verification does not guarantee coverage, payment, or reimbursement.
Life Stone makes no representations or warranties, express or implied,
regarding the accuracy, completeness, or enforceability of information
provided by insurance companies. Insurance carriers may change,
reinterpret, or retroactively apply benefit determinations, including
denials, medical necessity determinations, or payment adjustments, after
services have been rendered.
Final
determination of coverage and payment is made exclusively by the
insurance company at the time a claim is processed. Clients are
financially responsible for all charges
not paid by insurance, including copayments, coinsurance, deductibles,
denied claims, non-covered services, retroactive denials, or
recoupments—regardless of any prior verification or estimate provided by
Life Stone.
Clients are encouraged to independently
verify benefits directly with their insurance carrier and obtain written
confirmation when possible.
MEDICAL NECESSITY DETERMINATIONS
Insurance reimbursement is subject to medical necessity determinations made by the insurance carrier.
While services may be clinically appropriate and recommended by a Life
Stone provider, Life Stone does not guarantee that services will be
deemed medically necessary or reimbursable by an insurance plan. Clients
remain financially responsible for services denied based on medical
necessity determinations.
OUT-OF-NETWORK INSURANCE
For
services provided under an out-of-network insurance plan, payment is
due in full at the time of service, unless prior written arrangements
have been approved by Life Stone.
Upon request, Life Stone may provide a detailed receipt or superbill for services rendered. Submission of this documentation to an insurance carrier does not guarantee reimbursement.
Any reimbursement determination, including amount and timing, is the
sole responsibility of the insurance carrier and the client.
SELF-PAY & LOW-COST SERVICES
Clients who elect not to use insurance, or who do not have insurance coverage, may utilize self-pay services.
Self-pay rates vary by provider, service type, and duration and are
subject to change. For current rates with a specific provider, please
contact us.
Life Stone also offers reduced-cost therapy sessions at $80 per session with Master’s-level interns
for individuals who are uninsured or experiencing financial hardship.
Availability is limited, based on clinician capacity, and subject to
program guidelines.
Life Stone reserves the right to modify self-pay and reduced-cost offerings at any time.
THIRD-PARTY PAYMENTS
Life Stone may accept payment from approved third-party funding sources, including but not limited to:
Acceptance of third-party payment is subject to
verification, authorization, and compliance with applicable program
requirements. Clients remain responsible for any balances not covered by
third-party funding.
PAYMENT RESPONSIBILITIES
Payment for services is due at the time services are rendered,
unless prior written arrangements have been made. Life Stone may
maintain a payment method on file and may charge outstanding balances in
accordance with client authorization.
Clients are responsible for all financial obligations realted to services, rendered, including:
Accounts with unresolved balances may be subject to
additional billing statements, suspension of services, or referral to a
collections process, in accordance with applicable law.
Clients are responsible for fees associated with returned payments, failed transactions, or chargebacks, as permitted by law.
OVERPAYMENT & ACCOUNT CREDITS
In
the event of an overpayment, the excess amount will be retained as a
credit on the client’s account. Credits may be applied toward future
services unless the client submits a written request for a refund.
Clients
may request a refund of any credit balance at any time by contacting
Life Stone’s billing department. Approved refunds will be processed
within a reasonable timeframe, typically within 30 days, subject to banking and processing requirements.
If
no refund request is received and the client continues receiving
services or remains in communication with Life Stone, the credit will
remain active and available for use.
INACTIVE ACCOUNTS & UNCLAIMED CREDITS
In compliance with Utah’s Revised Uniform Unclaimed Property Act (RUUPA),
if a client’s account remains inactive and Life Stone is unable to
confirm the client’s intent to retain a credit balance, Life Stone may
be legally required to report and remit unclaimed funds to the Utah
State Treasurer’s Office after a dormancy period of three (3) years.
Prior
to remittance, Life Stone will make reasonable efforts to contact the
client using available contact information, as required by law.
An account may be considered active if the client:
Continues receiving services
Responds to appointment reminders
Accesses the client portal
Communicates with Life Stone in any form
Life Stone may send notifications to confirm credit
balances and client intent in order to comply with state law. Failure to
respond may result in remittance of funds as required by statute.
The following disclaimer is included in Life Stone’s Financial Agreement and applies to all clients:
In
the event of an overpayment, the credit will remain on your account and
may be applied toward future services unless you request a refund in
writing. If your account remains inactive and we are unable to confirm
your intent to keep the credit on file, we may be required by Utah law
to remit unclaimed funds to the Utah State Treasurer’s Office after a
period of three years. By continuing services or maintaining
communication, you help keep your account active and your credit in
place. You may contact us at any time to request a refund or to confirm
your preference to retain the balance.
