Client RIGHTS
At Life Stone Counseling Centers, we are committed to providing care that is respectful, ethical, and client-centered. As a client receiving mental health services in the state of Utah, you have rights protected under applicable state and federal laws, including rules established by the Utah Department of Health and Human Services (DHHS), such as Utah Administrative Code R501 governing licensed behavioral health programs.
Respect & Dignity
You have the right to:
Be treated with respect, dignity, and compassion at all times
Receive care in a safe, supportive, and professional environment
Be free from discrimination of any kind
Informed Consent & Participation
You have the right to:
Be fully informed about your treatment, including risks, benefits, and alternatives
Participate in decisions regarding your care and treatment goals
Refuse or withdraw consent for treatment, except where prohibited by law
Be informed of provider credentials and qualifications
Privacy & Confidentiality
You have the right to:
Have your personal and health information protected in accordance with Health Insurance Portability and Accountability Act
Review and request copies of your records, subject to legal limitations
Request restrictions on how your information is shared
Access to Care
You have the right to:
Receive services in a timely and appropriate manner
Be informed of fees, billing practices, and financial responsibilities
Request reasonable accommodations for communication, language, or accessibility needs
Quality & Ethical Care
You have the right to:
Receive care that meets professional and ethical standards
Ask questions and receive clear, understandable answers
Request a second opinion or referral when appropriate
Freedom from Harm
You have the right to:
Be free from abuse, neglect, exploitation, or harassment
Receive care in the least restrictive and most appropriate setting
Grievances & Complaints
You have the right to:
Voice concerns or file a grievance without fear of retaliation
Receive a prompt, fair, and confidential review of your concerns
Be informed of the outcome of your grievance
If your concerns are not resolved, you may also file a complaint with the Utah Department of Health and Human Services or the appropriate professional licensing board.

Participate actively in your treatment
Treat staff and others with respect
Follow agreed-upon policies and treatment recommendations
